The following physicians are available to continue your Obstetric and Gynecologic care.

* Dr. Katrina Lake, SPI physician group, Gynecologic services only

* Dr. Shaunda Chin-Bonds, Duly Medical Group, Gynecologic and Obstetric services

* Dr. Dexter Arrington, Duly Medical Group, Gynecologic and Obstetric services

As of April 1, 2025, I'm glad to leave my patients in one of these physicians' capable hands.

Of course, you may seek medical care from another doctor. If so, I recommend looking for a new physician as soon as possible. Your insurance company can help you begin your search by doctors who are accepting new patients.

Your medical records are confidential, and a copy can be transferred to another doctor or released to you (or to another person you designate) with your written permission.

You can easily access your medical records via the Athena EMR Patient Portal. Your new provider can also access your medical records via Care Everywhere in their office EMR. If you have not signed up for the patient portal, please do so ASAP. Any of my office staff can assist you with the process, it takes less than 5 minutes to sign up for the patient portal. You will need an email address or the email address of a trusted surrogate to sign up.

If you want a paper copy of your records, there is a charge (consistent with state regulations) for that service. Medical records must be picked up in person and the fee paid prior to copying. This process must be completed prior to April 1, 2025.

Unfortunately, after April 1, 2025, I WILL NOT be able to review/respond/address any of your medical conditions, labs or radiology studies. You will need to address those concerns with your new provider.

Please resolve any financial obligations or outstanding bills by July 1, 2025, to avoid your account being sent to collections.

It has been a pleasure providing care for you and I valued the trust and loyalty you placed in me. I wish you all the best in your future health. I will especially miss our delightful conversations and laughter.

We look forward to seeing you on your scheduled appointment date. If you are a new patient, information is being sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time, pre-register your demographic information and remind you to have a driver available to take you home after the appointment if needed. When you check-in for your appointment, we will need to copy your insurance card(s) in order to bill your insurance company.
If the office has closed, press 1 and leave your name and number. My staff will call you back on the next business day and schedule your appointment.
We use Athena electronic medical records. We encourage our patients to enroll in the EMR. Once you sign up you to have access to your medical records, correspond with the office on non-urgent matters, request an appointment and e-script prescriptions to your pharmacy. My staff will be happy to assist you with enrolling in the Athena EMR. 
I have Spanish speaking staff if Spanish is your preferred language.

Appointments & Cancellations

You may schedule an appointment with a patient care coordinator.

With the exception of serious emergencies, it is expected that you keep your appointments. If you need to reschedule an appointment, it is necessary that you advise the office at least 24 hours in advance. Your consideration in this matter will allow me to accommodate other patients who could benefit from an appointment.

Emergencies

I am available 24 hours for emergencies. Call the office number (708-755-3300) and press 3. Please follow the prompts.  If your concern is urgent but not an emergency, you can leave a message on your Athena patient portal or call the office (708-755-3300) press 1 and follow the prompts.

Payment

My office accepts the following methods of payment for co-payments, deductibles and services that are not covered by insurance:

  • Personal Check

For your convenience, we accept cash, check, Visa, Master Card and American Express.

We will bill your health insurance; you will receive an explanation of benefits (EOB) from your insurance company. Please check it carefully for accuracy. Your EOB will outline what charges are covered and your financial responsibility.
I also have a self-pay policy for patient without health insurance or for non-covered services.
